What information is needed to bill my extended health plan?
- The name of your insurance provider
- Your legal name as listed on the plan
- Certificate/ID number
- Policy/group number
- Date of birth
- Doctor’s note (if required by your plan)
Do you offer direct billing to ICBC?
Yes, we offer direct billing to ICBC. ICBC offers a predetermined amount to cover your treatments. If the treatment cost exceeds the amount covered by ICBC, the treatment will have a price listed as a User Fee.
What we need:
- Your legal name as listed on your Drivers License
- ICBC claim number
- Date of the accident
- BC Services number
- Claim Representative name and contact information
How much coverage do my benefits provide for this treatment?
Unfortunately, we cannot determine your coverage details.
At the time of payment, your insurance company will confirm if your claim is approved and the coverage amount. Please contact your insurance provider or review your policy to understand your eligibility.
Does Synapse Massage and Wellness direct bill secondary insurers?
Unfortunately, we do not bill secondary insurers at this time.
We will provide you a receipt in which you can use to submit to your secondary insurance plan.
**With the exception to Pacific Blue Cross – if you have Pacific Blue Cross as both a primary and secondary insurance plans – our system will automatically submit to both of your Pacific Blue Cross plans.**
